Regulatory Notes
The following Federal
Register post has been added to the U.S. Hazmat.Dot.Gov web site:
*Did you know that according to PHMSA (Pipeline and Hazardous Materials Safety Administration), “Most transportation incidents involving
hazardous materials are the result of human error. Training is the best means of preventing hazardous materials incidents.”1
Here are some additional examples of what happens when companies are
alleged to be out of
compliance with the Hazmat Regulations and are subsequently fined:
(Cylinder Retester). Failed to demonstrate the accuracy of the pressure-indicating device on the test equipment at a
point within 500 psi of the actual test pressure for pressures at or above 3,000 psi; failed to maintain complete and accurate records of cylinder
re-qualification; failed to provide recurrent employee training. [180.205(g)(3)(i),
180.215(b), 172.702(b) and 172.704(c)(2)]
FINE: $3,035.00.2
(Foreign Chemical Manufacturer). Offered ammonium hydrogendifluoride, solid, 8, and failed to (1) declare the material
as hazardous on a shipping paper, and (2) mark or label packagings to indicate the presence of hazardous material, thereby creating an undeclared
shipment. [172.200(a), 172.201 and 172.301(a)]
FINE: $18,050.00.2

Aviation Humor
After landing:
"Thank you for flying "XYZ" Airlines. We hope you enjoyed giving us the business as much as we enjoyed taking you for a ride."
In-Flight Announcement:
"Weather at our destination is 50 degrees with some broken clouds, but they'll try to have them fixed before we arrive.
Part of a flight attendant's arrival announcement:
"We'd like to thank you folks for flying with us today. And, the next time you get the insane urge to go blasting through the skies in
a pressurized metal tube, we hope you'll think of us again."
